IDENTITY CARD
1. OBTAINING IDENTITY CARD FOR THE FIRST TIME
* Please complete the following document and hand over to the grama niladhari
* Form 101 (Obtain from the grama niladhari)
* The original and photo copy of the birth certificate.
* 5 colored photographs which was the left ear clearly.(should be obtained from the recommended studio)
* Rs. 3/- stamps - between 16 - 17 years
Rs. 13/- stamps - over 17 years
* If employed the certificate from the employer in support of such employment.
* If a priest, the certificate from the viharadhipathi
* In case of photograph in school uniform, the certificate from the principal of the school.
* In case of one day service to be removed personally an envelop of adequate size should be handed over.
* If a age is over 20 years a report showing the cause for delay.
* These charges are liable be amended from time to time.
* On completion of all these requirements inquire the date on which you can obtain the identity card.
2. OBTAINING A REVISED IDENTITY CARD
* Please complete the following document and hand over to the grama niladhari
* Form 108 (Obtain from the grama niladhari)
* The original birth certificate.(Need not for the recent identity card)
* 5 colored photographs which was the left ear clearly.(should be obtained from the recommended studio)
* Rs. 15/- stamps
* If employed the certificate from the employer in support of such employment.
* If a priest, the certificate from the viharadhipathi
* In case of one day service to be removed personally an envelop of adequate size should be handed over.
* On completion of all these requirements inquire the date on which you can obtain the identity card.
BIRTH/MARRIAGE/DEATH CRETIFICATE
1. INSERT OR ALTERNATE A NAME IN THE BIRTH CERTIFICATE/ALTERATION OF SURNAME
* Applying letter
* Relevant application
* How to forward the application:
If the age is less than 21 years - Mother/Father/Guardian
If the age is more than 21 years - Applicant
* Mother's/father's birth certificate is required for the ages less than 7 years.
* Submit one of the following documents in support of the use of altered name for the ages more than 7 years.
Pupil's record
Grama niladhari's report
Birth certificate of a brother (alteration of surname)
If married ; Marriage certificate
Paper advertisement
A bank pass book
2. REGISTRATION OF PAST DEATH
* Applying letter
* Relevant application
* Nil result to the effect that the death has been registered.
* Grama niladhari's certificate.
* If the death occured in hospital copy of the original document of the hospital.
* Statements of indolence / death notices.
* A statement from a participant at the funeral.
* Name and address of viharadhipathi who conducted religious observances at the funeral.
STAMP FEE
* If the date and number of registration is known stamps of Rs. 25/= for a copy are required.
* If the date and number of registration is unknown stamps of Rs.50/= for a copy are required for searching a period of 03 month.
* For searching a period of 02 years Rs. 100/= for each copy.
